Job summary

Community Options, Inc. seeks Community Integration Counselors in the Vail, AZ area to provide individualized supports for participants with disabilities in the community. The role emphasizes promoting independence and coordinating services with a case manager.

Responsibilities include meeting regularly with participants, administering medications, and coordinating transportation and community activities while maintaining proper documentation. The position offers an hourly wage of $15.00.

Qualifications

• Must have High School Diploma or GED.
• Valid driver’s license with clean driving record is required.
• Minimum 90 days of relevant experience supporting individuals with intellectual and developmental disabilities.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Ability to exercise good judgment and remain calm in crisis situations.

Responsibilities

• Support individuals to increase independence.
• Meet with waiver participants regularly at home, community, or office.
• Collaborate with Case Manager to develop programs meeting social, emotional, physical, and personal needs.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration goals.
• Assist in building a community support network.
• Ensure physician-signed medical forms are completed for appointments.
• Administer medications and document appropriately.
• Coordinate leisure and volunteer opportunities.
