Job summary

Socket.dev in Garden City, NY is seeking a Community Habilitation Specialist to participate in the lives of individuals in their homes and support growth toward independence with a person-centered approach.

The role requires a high school diploma or GED, some experience with developmentally disabled individuals, access to a personal device for documentation, and a preferred NYS driver’s license. CPR certification is a plus, with frequent community travel as needed.

Qualifications

• High School Diploma or GED.
• Some prior experience on a personal level with developmentally disabled people.
• Must have a personal computer device, such as smartphone, or tablet, and be able to perform required documentation on the personal device using company’s software.
• Unrestricted NYS Driver's License (Preferred) and proof of car insurance needed if client’s goals require the use of a vehicle.
• CPR Certification (Preferred).

Responsibilities

• Participate in the life of the individual in their home to foster a person-centered atmosphere conducive to growth and independence.
• Commit to person-centered supports and honor the personality, culture, and strengths of the individuals.
• Promote and protect the health, safety and emotional well-being of the individuals.
• Assist in planning and implementation of the program.
• Maintain daily progress reports, datasheets, timesheets and other required documentation.
• Document training sessions in databases and report progress and barriers with each client.
• Attend staff trainings and meetings as required.
• Protect individuals' rights at all times.
• Maintain contact with Supervisors, individuals, and family about schedule changes and issues.
• Communicate relevant information to Supervisors verbally and in writing.
• Complete documentation electronically as required.
• Follow EVV schedules and policies for incident reporting.
• Accompany and advocate for individuals in the community.
• Travel to borough assigned as needed.
• Act as a role model and support individuals as outlined in ISPs.
• Meet with individuals several times per week per coordinated hours.
• Provide personal care as outlined in habilitation plans (bathing, toileting, dressing, laundry, etc.).
• Support the mission and vision of the organization.
• Perform other duties as required by supervisor or organization.
