Qualifications

• Minimum of 18 years old.
• Pass and maintain a passable criminal background check.
• Pass required controlled substance tests.
• Ability to speak, read and write effectively in English.

Responsibilities

• Maintain punctuality and attendance at worksite.
• Stay awake, responsive, and communicate effectively.
• Teach individuals daily living skills.
• Plan appropriate activities for individuals.
• Create opportunities to increase quality of life.
• Provide a clean, healthy, and safe environment.
• Implement Person Centered Support Plans.

• Assist individuals with medication.
• Complete necessary administrative tasks.
• Maintain communication with the Manager.
• Follow Provider Code of Conduct policies.
• Other duties as assigned by the supervisor.
