• Perform day-to-day operations of Integrated Case Management for high-risk members.
• Develop and monitor Case Management Care Plans with members.
• Collaborate with medical and behavioral providers for optimal member care.
• Work with high-risk patients to help identify needs and goals.
• Understand and comply with NCQA guidelines and HEDIS measures.
• Assist with discharge planning and special clinical projects.
