Job summary

Compass Career Solutions, sister company to Dungarvin, seeks an Employment case manager to help individuals with intellectual/developmental disabilities and mental health diagnoses gain and maintain employment. This in-person role emphasizes assessing eligibility, documenting progress, and coordinating with community partners.

You will support clients in achieving employment goals, document milestones, and participate in ongoing training and meetings.

Qualifications

• High school diploma or GED; preference for a Bachelor's in social services.
• Must be at least 21 years old.
• One-year related experience in employment services or crisis work.
• Valid driver’s license, insurance, and access to a vehicle.
• Self-starter with strong time management and problem-solving skills.
• Ability to work a flexible schedule and travel as needed.

Responsibilities

• Maintain knowledge of DDA and DVR programs and eligibility criteria.
• Interview clients to determine eligibility for employment services.
• Document care, follow support plans, and collaborate with internal/external teams.
• Identify client employment goals and employer matches; support growth and skill building.
• Provide ongoing support to help clients obtain and retain employment.
• Document progress in service plans and barriers to employment.
• Build and sustain community partnerships with providers and employers.
• Represent Compass at meetings and community events; attend internal/external meetings.
• Travel throughout the area; provide transportation as needed.
• Utilize assigned technology (laptop, phone, software) in the field.

Job Description

• Maintain an in-depth knowledge of DDA (Developmental Disabilities Administration) and DVR (Division of Vocational Rehabilitation)
• Conduct assessments and interviews with clients to determine eligibility for employment services.
• Good communication is key in this role. You’ll rely on your reading, writing, and communication skills to document care, follow support plans, and collaborate with both internal and external team members.
• Discover and maintain knowledge of clients, their employment goals, potential employer matches, and other case mgt. needs.
• Provide ongoing support to assist clients in obtaining and retaining independent employment.
• Assist clients in furthering their career growth and skill building.
• Document data and notes on the individual’s progress toward goals on barriers to employment and in their service plans.
• Make connections to maintain strong community partnerships with other service providers and employers.
• Represent Compass at professional meetings, community events, and to members of the community.
• Attend and actively participate in scheduled internal and external meetings.
• Travel throughout the area and provide transportation to clients as needed.
• Ability to use assigned technology as directed in the community. Laptop, cell phone and designated software

Qualifications

• High school diploma or general education degree (GED) with a preference for a Bachelor’s degree in the social service field.
• Must be at least 21 years of age.
• One-year related experience and/or training working with people in crisis, specifically in employment services.
• A valid driver’s license, insurance, and access to a vehicle insured for you as a driver are required
• Self-starter, excellent time management skills, and proactive problem solver
• Ability to work a flexible schedule
• Technology savvy - Especially Microsoft Office Applications and Zoom. PC and Android devices.
• Goodcommunicationis key in this role. You’ll rely on your reading, writing, andcommunicationskills to document care, follow support plans, and collaborate with both internal and external team members.
• Ability to perform CPR/First Aide as needed
• Ability to lift up to 50 lbs.
