Job summary

Qualifications
- Active and unrestricted RN license in California.
- Minimum 1 year of recent Home Health experience.
- Current CPR certification.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
Responsibilities
- Conduct initial and ongoing assessments of patients' health status and needs.
- Develop and implement individualized care plans based on patients' conditions and goals.
- Administer medications, treatments, and procedures as ordered by physicians.
- Monitor and document patients' progress and response to care.
- Provide education and support to patients and their families.
