Job summary

LCMC Health in Louisiana seeks a Substance Use Navigator to identify, engage, and support patients with SUDs in the emergency department. You will promote access to MAT, harm-reduction strategies, and seamless connections to outpatient care and community services.

The role requires strong interpersonal skills, cultural humility, and the ability to work with patients facing homelessness, mental health crises, and social barriers. Collaboration with clinical teams is essential.

Qualifications

• Minimum of two (2) years of experience working with individuals with substance use disorders or related vulnerable populations in healthcare, community or social services settings.
• Experience in an emergency department or hospital setting is preferred.
• Experience coordinating care, navigating health systems, or providing patient navigation/case management is preferred.
• Lived experience with recovery may be considered in lieu of formal experience where appropriate.

Responsibilities

• Identify patients with SUD or co-occurring mental health disorders in the ED.
• Establish rapport and perform initial assessments and brief interventions.
• Advocate for low-threshold access to MAT and connect patients to MAT-capable clinics.
• Facilitate initiation of MAT with hospital clinicians and participate in quality improvement projects.
• Use motivational interviewing to communicate respectfully and culturally.
