The Providence Center is seeking a qualified behavioral health professional to join a multidisciplinary team providing psychosocial assessments and crisis evaluation for consumers in our care and in community settings.
You will perform comprehensive assessments, collaborate with other providers, and develop clear, consumer-centered care plans while maintaining strict confidentiality and professional conduct.
Work on a multidisciplinary team providing behavioral health and crisis assessment to consumers at The Providence Center and in the community.
Perform a comprehensive psychosocial assessment of assigned consumers.
Conduct level of care and crisis assessments to clients and non-clients in a variety of settings including homes, shelters, street outreach, etc. Response to active police / fire scenes required with safety protocols.
Collaborate with other providers, agencies, and individuals in the consumers' network of care.
Perform record keeping in accordance with Health Information Services and other Center requirements.
Attend trainings, case presentations and conferences.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
OCCASIONAL DUTIES: Serve on appropriate Center committees.
Provide mental health consultation to other community agencies.
Master’s degree in Counseling/Social Work or related field or Registered Nurse (RN). License preferred. QMHP eligibility required.
