Job summary

Qualifications

• Requires High School Diploma or GED.

Responsibilities

• Engage ED patients with substance use concerns using trauma-informed, person-centered approaches.
• Perform SBIRT activities and educate patients on treatment options.
• Facilitate referrals and warm handoffs to MAT providers, recovery services, and community resources.
• Coordinate with ED clinicians and social work team to support care plans.
• Document interactions and referrals accurately in the EHR.
