Job summary

Aviator Home Health is seeking a licensed Physical Therapist to join our clinician-led team in the Dallas/Fort Worth area. You will perform initial and ongoing assessments (including OASIS), develop personalized plans of care with physicians, and deliver high-quality home-based therapy to patients.

We offer a strong base salary ($90,000–$120,000) with a productivity model and generous bonuses, plus comprehensive benefits, PTO, and professional growth opportunities.

Qualifications

• BS in Physical Therapy (or higher) from an APTA-approved program.
• Active Texas PT license and valid Texas Driver’s License.
• Experience in home health preferred; EMR knowledge helpful.

Responsibilities

• Perform initial and ongoing assessments (incl. OASIS) to identify needs and eligibility.
• Create and update personalized plans of care with physicians and care team.
• Deliver therapeutic treatments and assess equipment needs.
• Track outcomes and coordinate discharge planning.
• Apply infection control and standard precautions.
• Supervise PTAs and HHAs as appropriate.
• Document accurately and timely; meet CE requirements.
