Basic Functions

This is a non-exempt position. Incumbent is responsible to provide services which may range from information, assistance forms, guides and refers clientele in need to the appropriate human service which may possibly alleviate that specific need. Acts as community/ district liaison for case management purposes, developing, implementing and monitoring a comprehensive combination of services to help clientele maintain independence in his/ her own home. Include MAC/TSOA liaison and case worker for the service area.
