Job summary

Qualifications

• MSW from a CSWE-accredited program.
• Must be licensed or certified to independently practice social work at the master's level.
• Experience with HUD-VASH or homeless services is preferred.
• Strong knowledge of social work assessment and treatment planning.

Responsibilities

• Provide clinical psychosocial and case management services for HUD-VASH veterans.
• Coordinate care with VA Medical Center, community health programs and housing authorities.
• Engage in outreach and cross-system collaboration to ensure continuity of care.
• Travel to patient homes and outreach sites as required by program.
