Qualifications

Responsibilities

• Provide safe patient care by maintaining organizational skills to coordinate delivery of quality care for assigned patients/families.
• Develop a culturally competent plan of care identifying problems, outcomes and preventative measures.
• Perform systematic patient and family assessments relevant to practice settings.
• Deliver age-appropriate care and evaluate effectiveness of care with plan adjustments as needed.
• Demonstrate professional behavior and collaborate with the health care team to ensure regulatory compliance.
