Qualifications

Responsibilities

• Assessment: performs multi-system assessments including biophysical, psychosocial, and discharge planning needs.
• Planning: develops individual nursing plan of care and coordinates discharge planning.
• Implementation: administers medications, follows physician plan, documents in EMR, and uses teaching plans.
• Evaluation: evaluates outcomes and participates in unit quality assurance activities.
