Qualifications

  • Graduate from an accredited School of Practical Nursing.
  • Four years nursing experience.
  • Current US state LPN license with authorization to practice in GA.
  • Current BLS certification. ACLS/ALS and PALS required within nine months of hire.
  • CPT training within 9 months of hire.
  • Stroke education/training as mandated by policy.

Responsibilities

  • Coordinate the continuum of care for ED referrals and facilitate radio and phone communications.
  • Obtain and update documentation in the EMR including patient history, diagnoses and transfer details.
  • Consult with ED physician and charge nurse to assess severity and determine ED acceptance of transfers.
  • Assist with admissions by coordinating with transfer facilities, bed coordinators and supervisors.
  • Ensure timely follow-up on labs, imaging, and prescriptions; provide referrals as needed.
