Camino Real Community Services seeks a Service Coordinator for Early Childhood Intervention in Seguin, TX. You will coordinate services for young children with developmental delays, liaise with families and providers, and help implement IFSPs to support developmental outcomes.

You will maintain caseloads, monitor progress, document activities to meet state regulations, and advocate for families while collaborating with multidisciplinary teams in a supportive community setting.

Qualifications

• Bachelor’s degree in a related human services field is required.
• Experience with young children and families, preferably in ECI or special education.
• Knowledge of early childhood development and disabilities.
• Strong organizational and communication skills.

Responsabilités

• Conduct initial and ongoing assessments of needs for children and families.
• Maintain a caseload providing case management services.
• Develop and monitor IFSPs with families and teams.
• Coordinate and schedule services with therapists, educators, and providers.
• Provide families with information, support, and advocacy.
• Maintain documentation meeting state and federal regulations.
• Facilitate communication among stakeholders to resolve issues.
• Participate in team meetings and professional development.
