Qualifications

• Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title)
• Two years of experience as Social Worker 1 or a Forensic Social Worker 1 and a bachelor’s degree in social work or social welfare
• Two years of professional experience providing direct public or private social work services, and a bachelor’s degree in social work or social welfare
• A master’s degree in social work or social welfare

Responsibilities

• Counsel students with vocational guidance, referrals, and advocacy to support transitions to employment
• Gather and evaluate educational, medical, vocational, and personal information to identify student needs and goals
• Coordinate resources by connecting students and families with community agencies and services
• Collaborate with educators, agency staff, and families to develop transition plans for students with disabilities
• Assist with rehabilitation service applications, compile documentation, and prepare information for eligibility determinations
• Deliver presentations, participate in meetings and events to promote awareness of vocational rehabilitation services
