Job summary

Cadence Care Network in Columbus, OH is seeking an Early Intervention Service Coordinator to support infants and toddlers with disabilities and their families. You will help develop and implement IFSPs, coordinate services, and connect families with needed supports.

The role involves collaborating with community providers, attending weekly meetings, and participating in trainings to improve continuity of care within Early Intervention services.

Qualifications

• Bachelor's degree in a human service related field required.
• Experience working with children ages 0–3.
• Valid Driver License and access to a vehicle for work.

• Take primary responsibility for the development and implementation of the Individual Family Service Plan (IFSP).

• Attend weekly team meetings with community providers to ensure continuity of care and participate in trainings with community partners related to Early Intervention.
