Job summary

PRIDE Health is seeking a travel Home Health Physical Therapist for a travel assignment in Red Bud, Illinois. The position runs for 17+ months with a 40-hour workweek and 8-hour day shifts, Monday through Friday, in Red Bud, IL.

Responsibilities include evaluating patients, creating care plans, delivering interventions, and coordinating with the care team. Requires an active PT license and at least 1 year of home health experience; experience with HCHB and Oasis is a plus.

Qualifications

• At least 1 year of home health experience is required.
• Experience with HCHB and OASIS is a plus.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop and adjust individualized care plans to improve mobility and function.
• Deliver skilled interventions including therapeutic exercise and gait training.
• Educate patients and caregivers on home exercises and safety.
• Document evaluations, visits, progress, and discharge summaries in EMR.
• Coordinate care with physicians and interdisciplinary team members.
