Job summary

Qualifications

Responsibilities

• Assess patients’ physical, social, and psychological status and gather information from the care team.
• Plan individualized care with patient, family, and team involvement; coordinate education and referrals.
• Implement care by directing the interdisciplinary team and managing priorities, time, and resources.
• Evaluate patient responses and adjust the care plan based on outcomes and satisfaction.
• Promote professionalism, ethics, documentation accuracy, and compliance with licensure and regulatory standards.
