Job summary

Sweetwater Care in Fresno, CA is seeking a Licensed Physical Therapist to consult with patients, diagnose movement dysfunction, and develop individualized treatment plans. You will assess progress, adjust treatments, communicate with the health team, provide discharge summaries, and ensure compliance with regulatory requirements.

Qualifications

• Licensed Physical Therapist (state licensure required).
• Ability to supervise PT assistants, rehab aides, and students.

Responsibilities

• Consult with patients to learn about their condition and review medical history and referrals.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track progress toward goals.
• Use standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate treatment outcomes and adjust services accordingly.
• Provide counseling to patients and families and coordinate with other health providers.
• Communicate with supervisor and health team about patient progress and plans.
• Provide comprehensive discharge summaries as required.
• Document all required information in electronic systems and maintain patient records.
• Participate in trainings and required in-services.
• Able to supervise and support staff and students.
• Adhere to company policies and code of conduct.
