Job summary

The PT will educate residents and families, coordinate DME needs, and contribute to discharge planning while maintaining high professional standards and compliance with organizational policies.

Qualifications

• Graduate of an APTA accredited program or equivalent.
• Licensed Physical Therapist in New Jersey.
• BLS/CPR certification.

Responsibilities

• Delivers therapeutic services and treatments per plan of care.
• Maintains safe environment and proper body mechanics during therapy.
• Performs documentation including progress notes, reassessments, and discharge summaries.
• Develops and updates plans of care and discharge plans as needed.
• Assesses need for DME and coordinates fitting with residents and staff.
• Educates residents and families about treatment plans and safety considerations.
• Communicates effectively with residents, families, and care team.
• Participates in discharge planning and continuous quality improvement activities.
