Job summary

Synchrony is seeking a licensed Physical Therapist to provide a full range of PT services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment. You will evaluate patients, design individualized plans, and implement exercises and modalities to restore function.

Must hold a current state PT licensure ( CPR preferred) and maintain documentation in compliance with facility policies.

Qualifications

• Must have and maintain a current state PT licensure or certification.
• CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans, including exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, body mechanics, and self-management.
• Maintain accurate documentation of patient progress and outcomes in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
