Job summary

SnapCare in Santa Fe, NM is seeking a Physical Therapist for a full-time position. The role involves 8-hour day shifts from 8:00 AM to 5:00 PM, five days per week with a weekend Fri-Tue rotation, and no on-call requirements.

The incumbent will float to Inpatient Rehab/Acute settings and wear the uniform provided. You will perform comprehensive evaluations, develop treatment plans, maintain detailed documentation, participate in conferences, counsel patients and families, and assist with student

Qualifications

• Current New Mexico Physical Therapist licensure required.
• BLS certification from AHA required.
• Proficiency with all modalities and equipment used by a Physical Therapist required.

Responsibilities

• Perform comprehensive diagnostic evaluations to determine physical and cognitive status of inpatients and outpatients.
• Develop and implement individualized treatment plans with measurable goals in collaboration with patients and families.
• Maintain thorough documentation including initial patient evaluations, daily and weekly progress notes, and discharge summaries.
• Participate in patient care conferences, family conferences, in-service programs, and departmental meetings.
• Counsel patients, families, and other involved individuals regarding evaluation, care, and treatment.
• Initiate and complete discharge planning in a timely manner and provide effective education and training.
• Assist with coordination and supervision of Physical Therapy and PT Assistant student programs.
• Assist with orientation of new associates and scheduling of patients to ensure quality coverage.
• Participate in Performance Improvement activities and maintain departmental productivity standards.
• Maintain awareness of community agencies to make appropriate referrals and increase knowledge of community resources.
• Demonstrate competence in age-specific, developmental, and population-specific patient care.
