Job summary

Qualifications

• US citizenship and English proficiency required.
• Graduate of ACPE-accredited school with BS Pharmacy or Pharm.D.
• Full, current pharmacist licensure in a US state/territory/DC.
• Preferred inpatient experience or PGY1 residency; licensure path for advancement.

Responsibilities

• Provide comprehensive pharmaceutical care in Ambulatory Care, Acute Care or CLC settings.
• Dispense medications and oversee distribution in assigned area.
• Monitor pharmacotherapy and perform drug therapy management.
• Counsel patients and conduct medication reconciliation.
• Participate in quality improvement and staff training.
