Job summary

Avancer Homes LLC is seeking a Direct Support Professional to assist Individuals with daily living activities and implement their personal plans. You will document outcomes and coordinate with the DT Administrator and interdisciplinary team to support safety and well‑being.

Role requires a High School diploma or GED, DSP certification or progress toward it, a valid Illinois driver’s license, and the ability to lift 50 lbs. Training and supervision are provided in a Developmental Training setting.

Qualifications

• No disqualifying criminal history; certified Direct Support Professional (DSP) or ability to become certified.
• Twenty One years of age or older; valid Illinois driver’s license and clean driving record.
• Ability to lift up to 50 lbs and maintain a safe, supportive environment.

Responsibilities

• Assist Individuals with activities of daily living to ensure proper care.
• Implement strategy outcomes and document tracking sheets.
• Provide supervision of physical and mental well‑being of Individuals.
• Maintain an abuse/neglect free environment and follow policies.
• Engage Individuals in activities and support transitions and safe transport.
• Attend trainings, in-services, and department meetings as required.

DT Direct Support Professional (DSP) General Function:

Ensure the completion of the strategy outcomes listed in each Individuals personal plan. Collect data on the Individuals on a timely basis as required. Document the Individuals tracking sheets as required. Provide supervision and oversight of the physical and mental well-being of the Individuals. Provide an abuse/neglect free environment for all Individual's served. Support the agency's Vision, Mission and Core Values.

DT Administrator

DT Direct Support Professional (DSP) Duties & Responsibilities:

• Assist Individuals with activities of daily living (eating, drinking, hygiene, dressing, etc.) as necessary to ensure proper care.
• Implement strategy outcomes for all Individuals.
• Document Individuals' strategy outcomes and issues.
• Provide input and feedback to the DT Administrator and the Interdisciplinary Team on matters concerning Individuals.
• Incorporate repositioning, toileting, and self-care into the daily routine.
• Request materials necessary for the completion of Individual's strategy outcomes.
• Engage Individuals in activities at all times.
• Assist in transition duties, which include but are not limited to, driving vans during AM and PM transition according to schedules. Ensuring individuals and equipment are secure for a safe transport. Assist Individuals into the buildings and homes.
• Participate in evacuation and emergency response procedures to ensure the safety of the Individuals served.
• Attend all in-services, training sessions and department meetings as required.
• Be knowledgeable and thoroughly adhere to and implement all policies and procedures.
• Ensure the rights and confidentiality of the Individuals served are protected at all times.
• Other duties as assigned by the DT Administrator or Executive Director

Educational Requirements:

High School diploma or GED

DT Direct Support Professional (DSP) Requirements:

• No disqualifying criminal history
• Certified Direct Support Professional (DSP)
• Medication Administration Personnel or the ability to become an administrator of medication
• Twenty One years of age or older
• Valid Illinois driver's license and ability to maintain a good driving record
• Ability to lift 50 lbs.

Work is performed primarily in the Developmental Training building, and occasionally in vehicles, meeting rooms both in the building and at other Agency sites.
