Job summary

The role focuses on helping individuals develop independence, follow care plans, and participate in community activities. Responsibilities include assisting with daily living, meal preparation, medication help as delegated, transportation, and maintaining accurate client records.

Qualifications

• Excellent interpersonal and written communication abilities.
• Ability to maintain accurate records of client progress and services.
• Ability to follow care plans and ISP schedules.
• Valid driver’s license may be required for transportation duties.

Responsibilities

• Supports individuals served by teaching skills for independence and community integration.
• Follows care plans written in the ISP with goals, schedules, and routines.
• Assesses skill level and provides appropriate assistance.
• Engages individuals in activities and monitors progress and services.
• Transports individuals to locations outside home/program area.
• Administers personal care and assists with meals and daily routines.
• Performs medication passing/monitoring as delegated by RN or supervisor.
• Maintains a safe environment and adheres to policies.
