Job summary

Healthfirst is seeking a Care Coordinator to support members navigating their health care system. You will work with a Care Management team or call center to provide care coordination and navigation services.

Key duties include handling inbound calls about benefits, documenting interactions in the PHI system, managing a member caseload for short-term coordination, and performing outreach to ensure services are in place. Strong communication and multitasking are essential.

Qualifications

• Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), or Registered Medical Assistant (RMA), or equivalent medical training/work experience.
• HS diploma/GED.
• Strong written and verbal communication with good grammar, spelling, and punctuation.
• Experience in a fast-paced environment managing multiple priorities.

Responsibilities

• Handle incoming calls from members regarding benefits, care coordination, program inquiries, connecting to their care manager, and screening assessments; document calls in designated PHI system.
• Manage assigned member caseload on care management teams; communicate with interdisciplinary care team and monitor needs to improve outcomes.

Job description

Care Coordinators are responsible for performing care management services in order to support members who require assistance in navigating their health care system. The Care Coordinator is aligned to a Care Management team or a call center.

Duties And Responsibilities

• For Call Center: Receive Incoming calls from members regarding benefit questions/issues, care coordination, program inquiries, connecting to their care manager, and screening assessments to identify risk factors requiring medical intervention. Contemporaneously and accurately documents calls in designated PHI system.
• For Care Management teams: Manages an assigned member caseload who require short term care coordination and health navigation. Works within the interdisciplinary care team to support timely communication of member issues or needs and monitors screening of members effectively to improve quality and cost outcomes.
• Performs outreach calls to members to confirm services are in place
• Additional duties as assigned.

Minimum Qualifications

• Certified Medical Assistant (CMA) or Certified Nursing Assistant (CNA) or Registered Medical Assistant (RMA) or previous relevant medical training or work experience
• HS diploma/GED
• Work experience requiring effective communication verbally and in writing while demonstrating good grammar, spelling, and punctuation skills
• Experience working in a fast-paced environment that requires handling multiple priorities simultaneously

Preferred Qualifications

• Associate’s Degree
• Prior work experience in a call center, care management, or healthcare environment or familiarity with medical terminology, Medicaid & Child health plus benefit products.
• Excellent telephone, organizational, and customer service skills
• Team player who is organized and detail-oriented

Hiring Range*

• Greater New York City Area (NY, NJ, CT residents): $48,600 - $65,960
• All Other Locations (within approved locations): $42,200 - $62,400

As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
