Northeast Kingdom Human Services (NKHS) is seeking a DS Service Coordinator in St. Johnsbury, Vermont, to support individuals with developmental disabilities and coordinate essential resources.

The role includes conducting screenings, planning individualized programs, advocating for services, and collaborating with families and providers to promote independence and well-being. This is a meaningful, community-focused position with a strong impact on lives.

Qualifications

• Experience working with individuals who are intellectually and developmentally disabled.
• Strong written and verbal communication skills.
• Valid driver’s license and automobile liability insurance.

Responsibilities

• Screenings and assessments: evaluate strengths, needs, and goals.
• Consumer and family orientation: inform about rights, services, and funding.
• Individual program planning and development: identify goals and support plans.
• Service coordination: advocate and provide resources and education.
• Crisis intervention: respond to acute needs and follow-up.

• Consultation: collaborate with other providers for quality care.
• Quality assurance: oversee plans and documentation.
• Reports and record-keeping: maintain timely records.
• Confidentiality: adhere to privacy regulations.
• Resource development: secure care and services and ensure housing safety.

RESPONSIBILITIES & SCOPE

• Screenings and Assessments: Evaluate strengths, needs, and set goals.

• Service Coordination: Advocate, provide resources, and education.
• Crisis Intervention: Respond and follow-up on acute needs.
• Referral: Identify and assist in accessing community resources.
• Consultation: Collaborate with other providers for quality care.
• Quality Assurance: Provide oversight, approve plans.
• Reports and Record-keeping: Maintain timely documentation.
• Confidentiality: Adhere to strict confidentiality regulations.
• Resource Development: Secure necessary care and services, ensure housing safety.

QUALIFICATIONS

• Education and Training Requirements
• High school diploma plus demonstrated experience.
• 1-2 years of related experience working with individuals who are intellectually and developmentally disabled highly preferred.
• QDDP and/or Bachelor's degree preferred.
• Competence in working with individuals who are intellectually and developmentally disabled
• Must have valid driver’s license, motor vehicle and automobile liability insurance coverage
