Job summary

  • Report all van maintenance problems to the Transportation Supervisor and Outpatient Manager.
  • Maintain the van log with updated addresses and phone numbers.
  • Communicate with the Outpatient Manager and staff regarding patient care issues.

POSITION SUMMARY

This position provides transportation of patients to and from the facility, providing continuous observation, interaction, and role modeling to the patients while maintaining safety at all times.

QUALIFICATIONS
  • Education: High school graduate or equivalent preferred.
  • Licensure: Maintains a valid CA driver's license and current medical examination.
  • Additional Requirements: Must be 21 years or older and have and maintain a good driving record as obtained from the Department of Motor Vehicles. Handle with Care and Verbal De-escalation training within 90 days of employment and CPR (training provided in-house). May be required to work flexible hours and overtime.
Qualifications
PRIMARY RESPONSIBILITIES
  • 1. Provide transportation for patients to and from the facility in a safe manner.
  • 2. Obey all traffic laws while driving company vehicles.
  • 3. Report all van maintenance problems to the Transportation Supervisor and Outpatient Manager.
  • 4. Responsible for the van log, ensuring that the address and phone numbers are updated.
  • 5. Communicate and consult with the Outpatient Manager and other staff members regarding patient care issues and information.
  • 6. Get lunch from the cafeteria and provide to patients, as assigned.
  • 7. Maintain a clean environment for patients at all times.
  • 8. Demonstrate responsibility and sound judgment regarding patient safety.
  • 9. Maintain appropriate therapeutic boundaries at all times.
  • 10. Address patients, visitors, and co-workers in a pleasant and respectful manner.
  • 11. Adhere to facility, department, corporate, personnel and standard policies and procedures.
  • 12. Attend all mandatory facility in-services and staff development activities as scheduled.
  • 13. Adhere to facility standards concerning conduct, dress, attendance and punctuality.
  • 14. Support facility-wide quality/performance improvement goals and objectives.
  • 15. Maintain confidentiality of facility employees and patient information.
  • 16. Demonstrate appropriate therapeutic boundaries. Consistently follow established procedures.

  4. The ability to competently and fully perform Handle with Care techniques is an essential position function.
  5. Must be able to hear sufficiently, with or without correction, to perform all essential job functions competently.
