Evolve Therapy Services, LLC is seeking a Director of Rehabilitation for an in-house Skilled Nursing Facility in Velva, ND. The role oversees all therapy operations, ensuring adherence to standards, regulations, and company policies while leading a team of therapists.

You will collaborate with the interdisciplinary team, manage PDPM/MDS processes, and maintain licensure requirements, with a focus on quality patient care and compliance. Excellent leadership and communication skills are essential.

Qualifications

• Degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology.
• Must have active (PT, PTA, OT, OTA, or SLP) in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure/certification.
• Experience with PDPM and MDS management preferred.
• Excellent verbal, written and interpersonal leadership and communication skills.

Responsabilités

• Oversee and manage all aspects of the therapy program in the assigned facility/facilities.
• Ensure knowledge of and compliance with regulatory requirements, company policies and documentation/billing standards.
• Establish facility-specific procedures with the interdisciplinary team and train therapy staff on procedures.
• Ensure therapy services meet regulatory requirements and company standards, including productivity targets.
• Communicate professionally with patients and families and address requests.
• Perform other duties as assigned to discharge responsibilities in the organization.

Job Position:

Director of Rehabilitation (PT, PTA, SLP, OT, or COTA) for an In-house Skilled Nursing Facility

Job Summary:

Is responsible for all aspects of therapy operations at the facility in accordance with established policies, procedures, and practices of the company under the direction of the Regional Director of Operations

Responsibilities of the Director of Rehabilitation:

• You will help provide the best possible therapy services to patients consistent with established standards of practice and company and department procedures, including:
• Responsible for oversight and management of all aspects of the therapy program in assigned facility/ facilities.
• Responsible for knowledge of, and adherence to all regulatory requirements, company policies, procedures, processes and guidelines, job description, certification requirements, HIPAA, confidentiality standards and resident rights.
• Responsible for establishing facility-specific procedures in collaboration with the facility interdisciplinary team, and for training all therapy staff on established procedures.
• Ensure all therapists provide therapy services in accordance with regulatory requirements, established standards of practice, Company policies and procedures, therapy department procedures and productivity standards.
• Ensure all therapists adhere to regulatory requirements and Company procedures regarding documentation and billing of therapy services.
• Communicate and interact professionally and respectfully with patients and family members in attending to and meeting their requests.
• Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.

Education & Requirements:
