Family Guidance Centers, Inc. in Chicago is seeking a NADP Coordinator to connect clients with treatment services in partnership with the CPD, CDPH, and local providers. The role emphasizes direct counseling, assessment, and discharge planning to support recovery and reduce recidivism.

The position is grant-funded and focuses on harm reduction, access to treatment, and warm hand-offs to ensure ongoing engagement in recovery within Chicago communities.

Qualifications

• High School diploma required; Bachelor's in Psychology, Social Work, or counseling preferred but not required.
• Master's degree preferred, not required.
• CADC certification preferred.
• Illinois LPC/LCPC or LCSW licenses preferred, not required.
• Current driver’s license and vehicle insurance.

Responsibilities

• Conduct biopsychosocial assessments.
• Develop and maintain diversion relationships with police districts.
• Assess substance use severity.
• Provide case management, discharge planning and aftercare coordination.
• Coordinate referrals to FGC and other providers per client needs.
• Counsel family members with patient consent to support treatment goals.
• Maintain at least 50% direct face-to-face counseling time.
