Qualifications

  • Bachelor's degree required.
  • 1–3 years’ healthcare discharge planning or care coordination experience.
  • BLS certification active or within 30 days of hire.

Responsibilities

  • Initiate, coordinate, and track referrals and transfers with accurate documentation.
  • Complete essential discharge workflows and regulatory steps, including Medicare notices and compliance tasks.
  • Coordinate post-acute services such as rehab, SNF placement, VNA, DME, and MassHealth forms.
  • Identify and escalate discharge barriers, including insurance, transportation, or placement delays.
  • Maintain relationships with post-acute providers and ensure up-to-date resource lists.
  • Facilitate access to behavioral health, housing supports, and community resources.
  • Assist with insurance coordination, prescription support, form preparation, collateral gathering, and notary needs.

The role facilitates post‑acute referrals, tracks referral progress, completes required documentation and regulatory notifications, and documents discharge activities to help prevent delays,support communication and improve patient flow.I Major Responsibilities:Support daily discharge planning by coordinating and communicating with care teams, identifying post ‑ acute options, and updating Estimated Discharge Date (EDD) and placement details.1) Initiate, coordinate, and track referrals and transfers as assigned while maintaining accurate documentation in required systems.2) Complete essential discharge workflows and regulatory steps, including Medicare notices and compliance tasks as assigned.2) Coordinate post ‑ acute services such as rehab, Skilled Nursing Facilities (SNF) placement, Visting Nurse Association (VNA), Durable Medical Equipment (DME), and MassHealth forms.3) Identify and escalate discharge barriers, including insurance, transportation, or placement delays.4) Maintain effective relationships with post ‑ acute providers and ensure up ‑ to ‑ date resource lists for care teams.5) Facilitate access to behavioral health, substance use treatment, housing supports, and community resources.6) Assist with insurance coordination, prescription support, form preparation, collateral gathering, and notary needs as required.All responsibilities are essential job functions.II Position Qualifications:License/Certification/Education:Required:1. Bachelor's degree2. One to three years ’ experience in a healthcare setting assisting with discharge planning, care coordination support, or a related health science role3. Basic Life Support (BLS) certification active or within 30 days of hirePreferred:1. N/AExperience/Skills:Required:1. Excellent verbal and written communication, collaboration and organizational skills. Ability to multi-task and follow-up independently.2. Working knowledge of common office software products (Outlook, Excel, PowerPoint)Preferred:1.
