Job summary

Mission Hospital in Asheville, North Carolina, seeks an Outpatient Discharge Planner to coordinate patient transitions to the next level of care. You will assess needs, collaborate with physicians, nurses, families and community providers, arrange post-discharge services, verify insurance coverage, and educate patients on their discharge plan to support continuity of care and reduce readmissions.

Qualifications include an Associate or Bachelor’s degree and timely completion of required

• Notary Public must be obtained within 6 months of employment start date.
• Associate Degree, or Bachelors Degree must be obtained within 30 days of employment start date.

Job Summary And Qualifications

The Discharge Panner coordinates patients’ transition from the facility to next level care, ensuring a safe and timely discharge. They assess patient needs, collaborate with physicians, nurses, families and community providers, arrange post-discharge services, verify insurance coverage, and educate patients on their discharge plan to support continuity of care and reduce hospital readmissions.

What qualifications you will need:
