Job summary

La Palma Intercommunity Hospital, part of Prime Healthcare, is seeking a Case Management professional to coordinate the Case Management Department under the direction of the Case Manager/Social Worker. You will assist with discharge planning, referrals, and Medicaid submission processes to support safe, patient-centered care.

The role requires LVN license and current BCLS certification, and it is a full-time daytime position within a community hospital setting.

Qualifications

Knowledge of Discharge Planning / Utilization Management / Case Management terminology in managed care environments.
Active LVN license required upon hire and maintain current.
Experience in basic to intermediate computer skills/knowledge.
Current BCLS (AHA) certificate required upon hire and maintain current.

Responsibilities

Coordinate the activities of the Case Management Department under the direction of the Case Manager/Social Worker.
Assist with development and implementations of discharge plans; assist with referrals and authorizations.
Maintain Medicaid state submission forms for presentation to on-site reviewer and file them accordingly.
Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe patient-focused discharge plan.
