Job summary

West Pines Behavioral Hospital in Westminster, CO is seeking a discharge planning coordinator to support inpatient and aftercare services. You will develop individualized discharge plans, arrange follow-up therapy, and coordinate with psychiatrists, social workers, and community resources.

The role emphasizes strong organizational and communication skills, experience in discharge planning, and the ability to work with patients and families to ensure smooth transitions.

Qualifications

• Associates degree in nursing or human services required; bachelor's degree preferred.
• One+ year of clinical experience preferred.
• Experience in discharge planning or care coordination preferred.
• Knowledge of community resources and levels of care preferred.

Responsibilities

• Develop discharge plans to promote relapse prevention and long-term health.
• Set discharge appointments for patients, including therapy with group, psychologist or psychiatrist.
• Coordinate with community resources to facilitate aftercare and provide documentation to providers.
• Act as liaison between referral sources, patients and counselors; ensure timely discharge documentation.
• Perform other duties as assigned.

Job description

West Pines Behavioral Hospital is a new 144-bed inpatient behavioral health facility serving Denver area residents located at 11455 Huron Street, Westminster, CO. A joint venture between Intermountain Health and Acadia Healthcare. Just opened in December 2024, the hospital provides comprehensive inpatient and intensive outpatient services to address the growing need for accessible, high-quality behavioral health care in the Denver metro area.

Pay range: $25.00- $28.00

Essential Functions

• Develop continuing after care plan, based on patient's needs, to promote long term health and relapse prevention.
• Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.
• Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
• Act as a liaison between referral sources, patients and counselors.
• Ensure all patient discharge planning documentation is clear, concise and timely.

Education/Experience/Skill Requirements

• Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.
• One or more years’ experience in a clinical setting preferred.

Preferred Qualifications

• Previous experience in discharge planning, care coordination, case management, behavioral health, social services, or a related healthcare setting preferred.
• Knowledge of community resources, behavioral health services, and levels of care preferred.
• Experience coordinating referrals, aftercare appointments, and community-based services preferred.
• Familiarity with psychiatric, substance use, or inpatient healthcare settings preferred.
• Strong organizational, communication, and interpersonal skills
• Ability to establish effective working relationships with patients, families, providers, and community partners.

Licenses/Designations/Certifications

• If applicable, current licensure as a LPN, RN within the state where the facility provides services.
• CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.
• First aid may be required based on state or facility requirements.
