Job summary

Universal Health Services in Orlando, FL is seeking a Discharge Planner to coordinate the transition of patients from inpatient behavioral health care back to the community. You will bridge clinical care and aftercare, ensuring continuity of treatment and reducing readmission risk.

The role involves arranging post-discharge services, meeting with patients to review plans, and ensuring access to prescribed medications.

Qualifications

• Bachelor’s Degree in social work or related field required.
• Two (2) years' experience working in a psychiatric setting preferred.
• Handle with Care, Verbal Deescalation and BLS/CPR required (May be obtained upon hire)
• Effective Oral and Written Communication Skills
• Must be available to attend the full hospital and departmental orientations

Responsibilities

Position Summary

The Discharge Planner is responsible for ensuring a smooth, safe, and effective transition of patients from inpatient behavioral health treatment back into the community.
