Job summary

MemorialCare Health System in Laguna Hills is seeking a Discharge Planner to coordinate safe, timely transitions for a designated caseload. This per-diem weekend/day role collaborates with the care team under the Care Manager and physician orders to ensure a smooth transition through the continuum of care.

The position emphasizes strong communication with patients, families, providers, and payors, plus knowledge of community resources and regulatory requirements to support quality discharge

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Licensed Vocational Nursing license preferred.
• 1-2 Years of discharge planning experience in an acute care hospital setting or skilled nursing facility preferred.
• Strong communication and care coordination skills.

Position Summary

This position requires full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The position is accountable for the coordination and execution of an appropriate and timely discharge plan for a designated caseload of patients. The discharge planner, under the direction of the Care Manager and order of the physician, collaborates with the patient/family and members of the health care team, to ensure a safe and effective transition through the continuum of care.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
