Job summary

Arrowhead Regional Medical Center (ARMC) is seeking a Discharge Registered Nurse to coordinate safe and efficient transitions from hospital to home. The role focuses on identifying eligible discharge patients and ensuring timely, well-communicated plans across care teams to optimize workflow and patient safety.

Qualifications

• Must possess and maintain Registered Nurse licensure with the State of California.
• One year of full-time equivalent experience as a licensed Registered Nurse.
• Must possess and maintain ACLS and BLS certification.

Responsibilities

• Prioritize safe patient discharges; Collaborate and communicate discharge care plans with the care teams.
• Identify and minimize delays relating to patient discharges; Evaluate patient’s readiness for discharge.
• Educate patients and their caregivers about post-discharge care, medication management, and follow-up appointments.
• Teach, delegate, and direct nursing staff and interpret patient needs to all persons participating in patient care.
• Maintain accurate records in the EHR of discharge plans and patient interactions.
