Qualifications

• Bachelor's degree or equivalent clinical experience in inpatient behavioral health.
• Three years of clinical inpatient behavioral health experience preferred.
• Unencumbered clinical license if eligible per state regulations.
• CPR and Handle with Care certification within 30 days of hire.

Responsibilities

• Facilitates the patient's continued recovery after discharge and coordinates aftercare.
• Creates and maintains up-to-date aftercare resources and community supports.
• Coordinates aftercare appointments with providers and therapists.
• Communicates with patients, families, and referral sources about follow-up care and services.
• Documents discharge summaries and contract details for each patient.
• Collaborates with physicians, nursing staff, and utilization review for continuity of care.
• Ensures timely completion of hospital and client documentation per standards.
• Applies crisis intervention skills and supports the team during urgent situations.
• Maintains confidentiality and adheres to HIPAA regulations at all times.
• Knowledgeable about clinical regulations including state and federal laws.

How you'll contribute:

Facilitates the patient's continued recovery after discharge. Provides aftercare plan coordination for patients who are in need of ongoing medical/psychosocial care following discharge. In collaboration with other members of the healthcare team, assesses medical needs of the patient and offers suggestions regarding follow-up care options.

JOB RESPONSIBILITIES

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge
• Creates and maintains up to date aftercare coordination community resources
• Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.
• Provides resource information to patients and families as appropriate for future care needs.
• Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.
• Creates a detailed Discharge Summary and Contract for each patient as assigned.
• Works well with the interdisciplinary team including physicians, utilization review and nursing staff members. Demonstrates active communication with team members.
• Completes all hospital and client documentation thoroughly, timely, and in accordance with hospital standards.
• Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team when crisis occurs, assisting others to ensure patient safety.
• Displays a thorough knowledge of confidentiality. Completes releases appropriately. Demonstrates knowledge of HIPAA regulations and abides by those standards.
• Demonstrates a working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws.

Qualifications and requirements:

• Education: Bachelor's degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
• Experience: Prior experience with psychiatric and chemical dependency patients.
• License: Unencumbered clinical license if eligible
• needed per state regulations preferred. CPR certification and Handle with Care certification within 30 days.
