Qualifications

  • Bachelor’s degree in a related field (or equivalent combination of education and experience)
  • QMHA Certification required
  • 2+ years of clinical experience
  • 1+ year of care coordination experience preferred

Responsibilities

  • Facilitates timely discharges and care transition support with providers and community agencies
  • Conducts brief functional needs assessments and develops service plans
  • Provides service coordination and supports Medicaid consumers during transitions
  • Maintains and distributes information about community resources
  • Participates in discharge planning and education for consumers/providers
  • Coordinates initiatives, tracks progress, and manages timelines
  • Performs complex data entry and data management
  • Ensures records and sensitive information are properly handled and archived
  • Develops and maintains documents, reports and templates
  • Receives and processes service requests and coordinates with the team
