Job summary

Firelands Health in Sandusky, OH, is seeking a dedicated case manager to lead the discharge planning process and coordinate care across medical staff.

Under supervision of the Director, you will monitor documentation, identify risk factors, and work with social work to ensure smooth transitions from acute care.

Applicants must be a RN or LSW/LISW with 3–5 years acute care experience; certification preferred.

Qualifications

• Active Ohio RN/LISW/LISW license required.
• 3–5 years acute care hospital experience.
• Experience with case management.
• Certification in related field preferred.

Responsibilities

• Conduct patient rounds with/without physician to assess needs and discharge risk.
• Review medical records for utilization and discharge plans.
• Coordinate discharge planning with multidisciplinary team.
• Communicate with attending physicians to ensure documentation accuracy.
• Identify barriers to safe discharge and coordinate referrals.
• Maintain regulatory compliance and quality improvement.
