This role requires travel within NY, using an agency vehicle; you will plan meetings, coordinate care, and train others in the planning process while documenting progress and outcomes.

Qualifikationen

• Bachelor's Degree in human services or related field required.
• 2 years of experience in foster care, child welfare, or case management required.
• Driver's License and ability to maintain insurability throughout employment required.

• Engage youth, caregivers, and county stakeholders with a strength-based approach.
• Plan and facilitate meetings; develop and coordinate customized plans.
• Monitor youth progress; provide ongoing guidance and resources.
• Collaborate with internal/external teams to meet individuals' needs.
• Lead, model, and train others in planning processes.
• Complete notes, documentation, and reports in a timely manner.
