Job summary

Responsibilities

• Provide strategic leadership for development and advancement of APC programs, care models, and innovation initiatives across PAFMG.
• Partner with physician, operational, and service line leaders to optimize APC practice and improve care delivery.
• Lead design, implementation, evaluation, and scaling of programs that improve access, quality, and patient experience.
• Develop standardized team-based care models and support consistent APC integration across service lines.
• Foster collaboration, innovation, and leadership development within the APC workforce.
• Participate in organizational leadership initiatives and meetings.
