Job summary

Allegria Village, located in Dearborn, MI, seeks a Social Work Director to lead the social services function for its skilled nursing community and ensure residents receive comprehensive psychosocial support throughout their stay.

The role requires collaboration with residents, families, nursing staff, physicians, and rehabilitation professionals to develop care plans, conduct assessments, coordinate discharge planning, and advocate for resident rights and quality of life.

Qualifications

• Bachelor's degree in Social Work is required; MSW preferred.
• Current Michigan social work licensure or appropriate credential required.
• Experience in long-term care, skilled nursing, or healthcare populations preferred.
• Previous supervisory or leadership experience preferred.
• Strong knowledge of psychosocial assessment, care planning, discharge planning, and community resources.
• Excellent communication, organization, documentation, and interpersonal skills.
• Commitment to person-centered care and resident advocacy.

Responsibilities

• Direct and oversee social work services for skilled nursing residents.
• Complete psychosocial assessments and develop individualized plans of care.
• Provide counseling, emotional support, crisis intervention, and advocacy for residents and families.
• Participate in care plan meetings and interdisciplinary team conferences.
• Assist residents and families with adjustment to the skilled nursing environment and changes in health or functional status.
• Coordinate discharge planning and collaborate with residents, families, healthcare providers, and community resources.
• Assist with transitions between levels of care and connect residents with appropriate services and resources.
• Address resident concerns and grievances in a timely and professional manner.
• Maintain accurate and timely documentation in accordance with regulatory and organizational requirements.
• Support resident rights, dignity, choice, and quality of life.
• Collaborate with nursing, rehabilitation, activities, admissions, and other departments to promote person-centered care.
• Provide leadership, guidance, and support to social services staff.
• Participate in quality improvement initiatives and regulatory surveys as appropriate.
• Maintain knowledge of applicable federal and state regulations governing social services in skilled nursing.
