Village Point, a Springpoint Senior Living Community in Monroe, NJ, seeks a Director of Social Services, Sub-Acute Rehab, to provide medically related social services and oversee care planning for residents.

The role focuses on case management, family updates, and coordination with staff to ensure high-quality psychosocial wellbeing and occupancy in a Sub-Acute Rehab setting.

Qualifications

• Bachelor's Degree required.
• CSW or LSW required.
• 3+ Years experience with Seniors.
• Experience in Long Term Care, state surveys and maintaining occupancy.

Responsabilités

• Develop and implement case management services to all residents.
• Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning and encouragement to participate in care planning.
• Maintain progress notes under federal and state guidelines for Sub-Acute Rehab.
• Assist staff to inform residents about health status and health care choices and their ramifications.
• Make referrals and obtain services from outside entities.
• Assist residents with financial and legal matters (pension, Medicaid, power of attorney, guardianship procedures).
• Arrange for provision of needed counseling services.
• Identify and support resident needs and preferences through assessment and care planning.

Village Point is seeking an experienced Director of Social Services, Sub-Acute Rehab, to join our team!

Village Point is a Springpoint Senior Living Community located in Monroe, NJ. At Springpoint, the Director of Social Services is responsible for providing medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial wellbeing of each resident.

Responsibilities:

• Develop and implement case management services to all residents.
• Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning and encouragement to participate in care planning.
• Maintain progress notes under federal and state guidelines for Sub-Acute Rehab.
• Assist staff to inform residents and those they designate about the residents' health status and health care choices and their ramifications.
• Make referrals and obtain services from outside entities.
• Assist residents with financial and legal matters (applying for pension, Medicaid, referrals to lawyer, obtaining Power of Attorney, legal guardianship procedures, trustee).
• Arranging for provision of needed counseling services.
• Through the assessment and care planning process, identify and seek ways to support resident's individual needs and preferences, customary routines, concerns, and choices.
• Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents' individual needs.

Education and Experience:

• Bachelor's Degree required
• CSW or LSW required
• 3+ Years experience with Seniors
• Experience in Long Term Care, state surveys and maintaining occupancy

Salary Range: $85,000 - $95,000 / year
