Job summary

Evolve Therapy Services, LLC in Annandale, VA seeks a Director of Rehabilitation (PT/PTA/SLP/OT/COTA) to lead the in-house therapy program at the skilled nursing facility. You will oversee operations, ensure regulatory compliance, and drive quality of care with the facility interdisciplinary team.

Responsibilities include establishing facility procedures, training staff, maintaining documentation and billing accuracy, and communicating effectively with patients and families.

Qualifications

• Degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology.
• Active license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Experience with PDPM and MDS management preferred.
• Excellent verbal, written and interpersonal leadership and communication skills.

Responsibilities

• Oversee all aspects of the therapy program in assigned facility/facilities.
• Ensure adherence to regulatory requirements, company policies, procedures, HIPAA, confidentiality and resident rights.
• Establish facility-specific procedures with the interdisciplinary team and train staff.
• Ensure therapy services meet standards of practice, policies and productivity targets.
• Ensure documentation and billing comply with regulations and company procedures.
• Communicate professionally with patients and families and address their requests.
• Perform other duties as assigned to support the organization.
