Job summary

Nursing and Rehab in Columbia County, FL is seeking a Director of Rehabilitation to lead and enhance the rehab department. This leadership role directs a skilled team of therapists and directly influences residents' quality of life.

The position oversees staffing, regulatory compliance, and development of individualized rehab plans, with a focus on outcomes and interdepartmental collaboration. A licensed therapist with leadership experience is essential.

Qualifications

• Bachelor’s degree in Rehabilitation or related field (minimum requirement).
• Active, unencumbered license as an OT, PT, or SLP.
• Minimum 3 years of rehabilitation experience.
• At least 1 year of supervisory or leadership experience in a healthcare setting (hospital or skilled nursing facility preferred).
• Strong knowledge of RUGs and CMI systems.

Responsibilities

• Direct, organize, and evaluate all rehabilitation services in alignment with facility goals.
• Ensure compliance with federal and state regulations; recommend and implement policy updates as needed.
• Collaborate with residents and families to develop and adjust individualized rehabilitation plans.
• Oversee daily staffing, scheduling, and resource allocation for rehab therapists and support staff.
• Build and maintain strong relationships with physicians, healthcare partners, and community organizations.
• Identify departmental challenges and develop effective solutions in collaboration with facility leadership.
• Ensure compliance during regulatory surveys and inspections.
• Oversee accurate and timely documentation of patient progress and treatment outcomes.
• Lead interdisciplinary team meetings to support optimal patient outcomes and continuity of care.
