JOB SUMMARY

The Director of Nursing oversees all day-to-day clinical operations within Sage Living, providing clinical leadership and direction, and ensuring compliance with policies and procedures, and all state and federal regulatory guidelines. Collaborates with the Executive Director to ensure quality care is delivered in a safe manner through effective and sustainable use of resources. Creates an environment that is consistent with quality outcomes, service excellence, and fiscal responsibility. Embraces the Eden Alternative culture of person-centered care.

ESSENTIAL FUNCTIONS
  • Service Excellence and Quality Outcomes
    • Is highly visible throughout Sage Living to develop positive relationships with residents, family members, and staff.
    • Directs the day-to-day operations of nursing staff to ensure each resident’s care plan is implemented and updated in a safe and timely manner.
    • Collaborates with the Sage Living leadership team, Sage Living staff, and across other St. John’s Health departments to ensure high quality care and resident outcomes.
    • Creates a culture of service excellence emphasizing and honoring resident choice and upholding Resident Rights.
    • Responsible for risk management and mitigation, and incident reporting with timely investigations and documenting the necessary follow-ups.
  • Regulatory Compliance and Resident Safety
    • Assists in the development and maintenance of Sage Living policy and procedures in Policy Tech. Assures that policies and procedures are in compliance with state and federal regulations.
    • Collaborates with the Executive Director in the effective implementation and compliance of all policies and procedure, and state and federal guidelines.
    • Plays an integral role in committees that directly impact resident care including: Fall Committee, NAR, our Restorative Nursing Program, QAPI Programs, and Infection Prevention Programs.
    • In Coordination with the Executive Director, participates in other committees and activities as requested, including but not limited to: organization-wide Leadership Team meetings, Performance Improvement Projects, and Sage Living staff meetings.
    • In collaboration with Care Coordinators, assists in care planning on medically complex residents.
  • Staff Development
    • Collaborates with the Executive Director on the care and staffing models for Sage Living necessary to meet resident needs and maintain department operations.
    • Responsible for effective recruitment, interviewing, hiring, on-boarding, and orientation for all team members.
    • Applies corrective discipline and/or coaching when needed to mitigate performance or behavior issues.
    • Responsible for 90-day and annual evaluations of Sage Living nurses, CNAs, and other clinical staff.
    • Responsible for developing, implementing, and monitoring training programs and in-services to ensure policy, state, and federal regulatory compliance.
    • Communicates with clinical staff on a consistent and ongoing basis to ensure staff are aware of resident care, policy, and/or regulatory changes.
    • Ensures the competency of staff is assessed, demonstrated and validated both annually and on an on-going basis.
  • Operational Efficiency
    • Collaborates with the Executive Director to identify the supplies and tools required to meet the nursing care needs of the residents.
    • Conducts routine environmental and clinical rounds to ensure regulatory compliance throughout Sage Living, that all neighborhoods are adequately stocked with the necessary supplies and equipment.
    • Proactively identifies areas of improvement, reviews products and supplies, and ensures optimal workflows to improve resident outcomes, staff efficiency, and/or resident and staff safety.
  • Teamwork
    • Builds a culture of collaboration fostering an environment that supports staff so they can provide excellent resident care.
    • Other duties as requested but not limited to, implementation of interdepartmental projects and programs, ensuring survey compliance and readiness, and assumes Sage Living leadership in the absence of the Executive Director.
JOB REQUIREMENTS

Minimum Education
Required: RN
Preferred: BSN

Minimum Work Experience
Required: Proven experience in a nursing leadership role.

Preferred: 2+ years of experience in a Long Term Care setting.

Required Licenses: Current RN license with the authority to practice in the state of WY and BLS required at the time of hire.

FUNCTIONAL DEMANDS
Working Conditions

Long term care and skilled nursing clinical setting. Occasional stressful situations, irregular hours. Care of residents with complicated medical/psychosocial conditions. May be required to take call shifts and must possess the ability to do occasional shift work. Potential overnight travel. Must be on-site at the SJH campus or other facilities to fulfill the responsibilities of this role.

Physical Requirements

Must be physically active including heavy lifting and moving of residents. Must possess the ability to push, pull or move residents, equipment and supplies. Must be able to lift up to 50 pounds.

Direct Reports: LPNs, RNs, CNAs, Care Assistant Trainees, MDS-Care Coordinator RNs, Restorative Aides, Medication Aides, RN Unit Educators and Clinical Coordinators.

Reports to: Executive Director of Senior Living.

Internal & External Contacts: All hospital departments, residents, patients, families, volunteers, physicians and members of the community.
