Job summary

Runk & Pratt Health Care Ent Inc. seeks an experienced nursing supervisor to lead direct care staff, oversee patient care delivery, and ensure compliance with physician orders and facility policies.

You will manage staffing, coordinate admissions and discharges, and serve as liaison with healthcare providers and families. A Virginia RN license and CPR certification are required, with long-term care experience preferred.

Qualifications

Job description

STAFFING:Provide leadership & supervision of all direct care staff dailyEnsure resident care is delivered in accordance with physician orders, care plans, and established standards of practice to maintain safe, high quality, resident-centered careMonitor direct care staff performance of health related activities, including the identification of any significant gaps in the staff person's ability to function competentlyAssist on the floor with routine needs, in the capacity as it relates to nursing or supervising issues. (Passing medications, documentation, issues with doctors or medical staff, coordination of appointments and therapies, assisting of families with their issues and needs)Oversee breaks and lunchtime of all direct care staff to ensure that proper coverage is always maintainedEnsure direct care staff wear follow dress code, have appropriate footwear, & wear proper identificationEnsure appropriate staffing levels to meet resident needs, manage scheduling (daily assignments and monthly schedule), and minimize overtimeKnowledgeable of the schedule for all Direct Care staff and monitor hoursRESIDENTS:Plan, implement and evaluate the residents daily and on-going needsOversee the daily needs & care of all residents, including hygiene and bathing issues, coordination with the dietary staff for resident meals, while also being aware of medical needs and monitoring medicationsBe proactive in building relationships with the residents so that they feel that they have an advocate and a support system within the facilityAdmissions: assist with setting up charts, assists residents and families with locating and settling into their room and orientating them to the facilityAssists resident and families during discharge ensuring all belongings are packed, extra medications are returned to the pharmacy and resident charts is broken down for filingMonitor clinical outcomes and implement quality improvement initiativesDOCUMENTATION:Review, monitor, and follow up on all daily documentation in the resident's file (functions, activities, illness, concerns, etc) completed by the RMA & aidesMaintain documents of all resident records & filing of such documentsPerform & ensure accurate and timely clinical documentation dailyEnsure a system is established to document and monitor nursing assessments, medical visits, and order changesReview & complete Incident Reports in a timely mannerComplete fall screenings upon admission, annually, and with any change in conditionComplete UAI / ISP / TB screening / Fall Screening on all residents upon admission and at least annually thereafter, and as needed, to maintain compliance & meet resident needsRecommend and implement, in writing, changes to a resident's UAI & or ISP (service plan) based on needs whenever the plan does not appropriately address the current health care needs of the residentMEDICATION:Monitor and audit medication carts, MAR logs and administration of resident medicationsPerform medication pass audits as outlined in the facility policyKnowledge of & an understanding of medication administration procedures and current medical issues facing the elderly populationEnsure all medication orders are accurately approved and followed in EHR system for administration per provider ordersImplement and follow all provider orders (including medications, labs, dietsOversee all narcotic/scheduled drug disposalsSUPERVISOR & GENERAL DUTIES:Carry out the daily routine as set forth by the Administrator in accordance with the needs of the facilityPerform rounds of the facility each morning and either contact the appropriate staff to deal with the areas found or report it to appropriate facility department to be correctedActs as a liaison & primary contact for all medical needs of residents including providers, hospitals, therapists, outpatient/outside vendors (hospice, home health, DME, etc)Act as the administrator when the Administrator is not present at the facility.
